From Photo consultation

Carefully fill out the form at the bottom of this page. Please read the rules for filling it out:

1. To receive an in-person consultation, please write your last name, first name, patronymic, age (full years), height, weight, contact phone number, email.

2. Attach to the letter recent photographs (NOT SELFIES) in standard projections (front, half-profile and profile on both sides), without flash, if surgery is planned on the body, then without underwear.

3. Write what you are not happy with about yourself, why you decided to have surgery and attach a photo or image of the desired result.

4. Briefly report on your health status: all existing diseases, medications you take, previous surgeries, whether you are registered with any doctor.

5. Indicate the name of the doctor whose consultation you would like to receive.

6. Make 100% payment for the service on the website.

7. After considering your application, within 14 days the doctor will draw up a conclusion, which will reflect the diagnosis with a detailed answer to the questions asked, and also give appropriate recommendations. The advisory opinion will be sent to you in writing by email.
